Medical Clearance for New Freshman/Transfer Registration (MMR and Meningitis Forms)

Under New York State law, every incoming student born after January 1, 1957 must be immunized against measles, mumps and rubella (MMR). The documentation or proof of immunization may be from your high school or your doctor, or your may have free MMR shots in the office of Health Services during our advertised clinic dates and times posted on our health care events page. A new public health law requires all colleges to distribute information about meningitis and the meningitis vaccine. You do not have to take the vaccine but must complete and return the form to Health Services, MC-02. If you decide to take the vaccine you may revise your information at any time. The form is available in the Health Services Office, MC-02 or you may download it from this website. Call 718-631-6375 for further information.
