Medical Scribe (MSCAT) (Ref# 139)

Description: A Scribe works closely with the provider to facilitate patient flow, to ensure an accurate and complete medical record for each patient for that physician and to transcribe healthcare data to provide better service and efficiency of clinical care. PRIMARY DUTIES - With patient permission, accompany physician into exam room to transcribe for the physician. - Document the physician dictated patient history, including history of present illness, review of systems, past medical and surgical history, family and social histories, medications and allergies. - List all proper diagnoses and symptoms as well as follow up instructions and prescriptions as dictated by the physician. - Transcribe patient orders including laboratory tests, radiology tests, and medications dictated by the physician. - Document physical examination findings and procedures dictated by the physician. - Document the results of laboratory and radiology studies as dictated by the physician. - Document the correct time of patient care related activities, including physician to physician communication, family communication and re-examination of the patient as dictated by the physician. - Make the necessary amendments when the physician concludes his/her review of all documentation completed by the Scribe. (The physician MUST review documentation of the patient's encounter and sign the chart). - All patient care must be communicated by the physician and not the Scribe. Scribes MUST refer all requests related to patient care to the responsible physician or nursing staff. ADDITIONAL RESPONSIBILITIES - Maintain and demonstrate an understanding of a team approach to patient care and documentation. - Treat all patient information and data transcribed in a confidential and secure manner. - Observes and complies with Boriken's risk management, safety and emergency policies and procedures. - Observes and complies with all local, state and federal guidelines for documentation into the electronic health record system. - Establish and maintain a professional working relationship with physician, staff and management.

Special Skills & Requirements Medical Scribes Certification and Aptitude Test I Certified (MSCAT) - Associates or bachelor’s degree in science preferred - 6 months to a year Medical Scribe experience in a clinical setting preferred - Basic knowledge in terminology of pharmacology, immunization, human anatomy medical terminology (spelling, terms and approved abbreviations), clinical diagnostic pathways including common tests, procedures, and medical work-ups. - Experience with eClinicalworks preferred. - Must pass medical terminology and dictation typing pre-employment test with the Medical Director or HR Staff - type 55 to 65 words per minute. - Must be proficient in the English language grammar and spelling - Bilingual English and Spanish - Ability to multitask efficiently and effectively.
