Case Manager - DR II

Description: The Case Manager provides services for adults living with HIV/AIDS, who reside in our 60-bed, HASA-funded permanent congregate facility. Responsibilities include completing comprehensive assessments and all required documentation, developing service plans and conducting at least weekly face-to-face visits with residents. The Case Manager develops a supportive relationship with clients, assists them in attaining their goals, ensures that clients pay rent and other bills, and links clients to community-based medical, mental health, and substance abuse treatment programs and all needed services.

Special Skills & Requirements • We are looking for a caring, supportive, organized and dependable case manager who finds it rewarding to work with clients with HIV/AIDS, many of whom have co-occurring substance abuse and mental health disorders. A team player who collaborates with clients, staff and community providers; the case manager should also possess excellent verbal, written and active listening communication skills and a flexible schedule. • Bachelor’s Degree in Psychology, Social Work, Human Services, related field.
