Research Foundation Hiring Procedures - New Hires

Research Foundation employee’s new hire package must be completed and sent to the Office of Sponsored Programs, Room A-507 at least one week before the proposed start date.

In order to expedite the processing of your new hire employee’s paperwork and payroll, have the following forms completed and signed:

  1. Employee Wage Title Designator - This form must be generated on the Research Foundation website by the Principal Investigator on or before the start date of the employee. Both the PI and Employee will need to sign.
  2. Letter of Appointment - No longer required, effective 4/11/13.
  3. Personnel Action Form (PAF) - Revised: 3/18/13

  5. Employee Personnel Data Form
  6. Emergency Contact Form
  7. Application for Employment
  8. Voluntary EEO Self Identification
  9. Consent to Request Consumer Report & Investigative Consumer Report Information (Revised) or Combined Disclosure Notice and Authorization Regarding Background Consumer Reports & Summary of Rights Under FCRA – Parental Consent for Minors Only
  10. 2013 Federal W-4 Tax Form - Revised: 1/31/13
  11. 2013 IT-2104 Withholding Allowance NYS or 2013 IT-2104 E Exemption from Withholding NYS - Revised: 1/31/13
  12. Direct Deposit Authorization Form or Chase Payroll Card Account Form
  13. Signed Group Retirement Plan Acknowledgement Statement
  14. Form I-9 Employment Eligibility VerificationRevised: 3/8/13 - Must be completed at the Office of Sponsored Programs, Administration Building, Room 507. Contact Ms. SueAne Solares-Loza for an appointment at 718-631-6357. If SueAne is not available, Ms. Laura Segura-Vicenti can also assist you with the completion of the Form I-9. We can only accept original, unexpired documents as proof of birth, identity and employment authorization. Please refer to the reverse side of the form I-9 for the List of Acceptable Douments. Form I-9 must be completed within three (3) business days of the actual start date of employment.

  17. QCC HR Approval Letter - Should only be completed if a prospective RF employee is currently a Full-Time/Part-Time CUNY Faculty/Staff.  Note that an additional Multiple Position Form will be required for Full-Time CUNY Faculty.

All new-hires are required to complete the Research Foundation's mandatory Online Harassment Training Program.
