Consumer Directed Personal Assistance Program (CDPAS)

Description: Lighthouse Guild has three health plans through which we manage every medical and healthcare need, especially for those who are visually impaired. We coordinate healthcare, therapy, in-home care and other services so that members can live safely at home. We are looking for Human Services students, who have an interest in social work and/or supporting social workers with the administrative portion of the client’s care plan implementation. They would use our telephonic model to follow-up with clients to ensure follow through on key portions of the client care plans. Consumer Directed Personal Assistance Program (CDPAS) – Students will assist Social Workers meet annual paperwork requirements, which allows members to recruit, hire, and supervise their own home care workers. CDPAS program requirements include an annual physician sign off. Under the direction of GuildNet social work staff, student will review client records for current physician signature then send paperwork out for update. Students will participate in staff meetings as they are available. Date: September 14, 2015 - May 31, 2016 Location: 111 East 59th Street or 15 West 65th Street in Manhattan

Special Skills & Requirements Qualifications: 1. Knowledge of Microsoft Word and Excel required. 2. Strong written and oral communication skills, including clear diction is required. 3. The ability to take and follow instruction, take initiative and apply critical thinking skills is required. 4. Prior experience where customer service skills have been used is preferred. 5. Experience managing a project independently then to completion is required. 6. The ability to exhibit excellence in the execution of assigned responsibilities is required. 7. Bilingual, Spanish, Chinese (Cantonese/Mandarin), Creole or Russian preferred.
