Passport Processing Services
In order to better serve the Queensborough Community and our neighbors, the Bursar's office is now accepting Passport Applications on behalf of the U.S. Department of State.

PASSPORT PHOTOS
Queensborough Community College regrets that it can no longer offer Passport Photo taking services. Please arrange to have your passport photo taken at your local retail establishment prior to presenting your application for processing. The US Department of State only requires one (1) passport photo per application.
Routine passport service takes between four (4) and six (6) weeks.

Passport Processing Fees (Effective 7/13/10):
Please note: CREDIT CARDS are not accepted for any Passport Services.
General Acceptance Fee - $25.00 paid to Queensborough Community College (QCC) by cash or money order (only) for all passport applications - this fee is paid by all applicants regardless of age, and CANNOT be combined or paid with the Department of State processing fee.
Applicants age 15 years and younger:
- Passport Book Only - $80.00 fee for all passport applications paid to the Department of the State by check or money order only.
- Passport Card Only - $15.00 fee for all passport applications paid to the Department of the State by check or money order only.
Applicants age 16 years and older:
- Passport Book Only - $110.00 fee for all passport applications paid to the Department of the State by check or money order only.
- Passport Card Only - $30.00 fee for all passport applications paid to the Department of the State by check or money order only.

FIRST TIME PASSPORT APPLICANTS
Passport requirements for all US Citizens 16 years of age and over:
- Proof of citizenship (birth certificate with a raised seal),
- Proof of identity (1 photo Id with name),
- 1 passport photograph (must be sized accordingly - see below),
- Completed form DS-11 - must be submitted in person with the applicant's signature witnessed by the College's passport agent,
- Married female applicants, who have assumed their husband’s surname, must provide marriage certificate with raised seal, and
- Applicable fees
Passport requirements for all US Citizens under 16 years of age:
- Proof of citizenship (birth certificate with a raised seal),
- Evidence of child's relationship to the parents or guardians,
- 1 passport photograph (must be sized accordingly - see below),
- Parental identification,
- Completed form DS-11 - must be submitted in person with the applicant's signature witnessed by the College's passport agent, and
- Applicable fees
If only one parent appears, you must submit one of the following:
- The non-applying parent must provide a copy of photo ID (Driver's License; Non-Driver Id Card; Passport) front and back;
- Notarized statement from the non-applying parent consenting to passport issuance for the child (form DS-3053, Statement of Consent);
- Primary evidence of sole authority to apply, or a written statement (made under penalty of perjury) explaining the second parent's unavailability;
- Court order granting sole custody to the applying parent/guardian;
- Adoption decree (if listing only the applying parent)
PASSPORT APPLICANT RENEWALS
Passport requirements for all US Citizens 14 years of age and over:
- 1 passport photograph
- Fees
- Completed form DS-82
- Mail the documents to: National Passport Center, PO Box 90155, Pittsburgh, PA 19190-0155
PASSPORT APPLICANT REPLACEMENTS
- Proof of citizenship (birth certificate with a raised seal),
- Proof of identity (1 photo Id with name),
- 1 passport photograph
- Fees, and
- Completed form DS-64 (Statement Regarding Lost or Stolen Passport).
