Is it ready already?? Ever wonder if you’ve completed all the required components of the IRB application? Please refer to the list below as a general guideline for what should be included in your application.
- Application - Includes original signatures and eight questions, which must be answered on a separate page.
- Consent form- You must first ask for your subjects’ permission in order to use any information in connection with your study. If you suspect some of your students may be under 18 years of age, you should include a signature line for parental consent within the form.
- Proposal- If you are conducting grant-funded research, you must include the grant proposal. If you are conducting dissertation/thesis research, you must attach a copy of your dissertation/thesis. If you are not conducting any of the above-listed types of research, a three-to-five page proposal that describes your research and its methodology is acceptable.
- Computer-based Training Certificate
- Copies - Submit 10 copies of all documents
