IRB Application & Instructions

The Queensborough Community College IRB, in keeping with CUNY guidelines, requires that researchers use the application listed below. Applications may also be picked up in the Administration Building, Room 504.  You can access the application materials at:

Continuing Review (Project Renewal)/Final Report Form

Applicants who have received IRB approval and would like to renew their studies beyond the approved one-year period must fill out the form below and check off the “Continuing Review” box. If you have completed your approved study, you must complete and submit the same form below and check off the “Final Report” box. While the IRB makes every effort to remind researchers of the one-year approval deadline, applicants are solely responsible for submitting this paperwork in a timely fashion. Failure to submit this paperwork will be considered non-compliance, and applicants should not work on their projects until it is submitted. If you are unsure when your study expires, please contact the IRB Administrator or refer to your approval letter, which provides the expiration date.

Serious Adverse Events Form

If a human subject experiences serious psychological, physical, or emotional distress during or after participating in a study, the Principal Investigator must report the incident to the home IRB and to CUNY’s compliance officer, Patricia MacCubbin. Principal Investigators must discontinue working on the project until the event can be investigated and the issue resolved. For this reason, the IRB recommends that applicants who are asking their subjects sensitive questions include a referral for on-campus counseling in the consent form.

IRB Application Checklist

Is it ready already?? Ever wonder if you’ve completed all the required components of the IRB application? Please refer to the list below as a general guideline for what should be included in your application.

  • Application - Includes original signatures and eight questions, which must be answered on a separate page.
  • Consent form- You must first ask for your subjects’ permission in order to use any information in connection with your study. If you suspect some of your students may be under 18 years of age, you should include a signature line for parental consent within the form.
  • Proposal- If you are conducting grant-funded research, you must include the grant proposal. If you are conducting dissertation/thesis research, you must attach a copy of your dissertation/thesis. If you are not conducting any of the above-listed types of research, a three-to-five page proposal that describes your research and its methodology is acceptable.
  • Computer-based Training Certificate
  • Copies - Submit 10 copies of all documents
